Telemedicine Notice:

Please note: All visits are scheduled through Telemedicine. The health care provider will meet you through the Stairway to Health Web-portal via your phone or computer. Let’s get together to determine your best healthcare needs.

If it is determined that you need to be seen, we will send you directly to the office for treatment.

Your Upcoming Appointment:

IMPORTANT: Please look for a text or email to complete your registration online. When your online registration is complete, please click a separate link at your appointment time to start your video visit. We can NOT start your video visit unless you have pre-registered online.

Here are a few tips about your upcoming visit:

Remember to have a valid ID, insurance card, and form of payment. If you have secondary insurance, please present your card at your appointment also.
You must complete your registration in advance. We regret we're unable to begin your video visit unless you've pre-registered.
Don’t forget to upload your insurance card and driver’s license and sign the consent to treat form online.
Please be on time, and if you need to cancel, use the link in your reminder text sent before your appointment.

1. Choose a reason and available appointment date & time

2. Book appointment and complete patient information

3. Confirm your identity with phone text or email

4. Complete medical consent forms and confirm/update Insurance Information, Address & Phone, Demographics. (1st visit ONLY)

5. Confirm/update insurance, credit card and demographic information

6. Check your email or phone text for Telemedicine appointment link

7. A link for your Telemedicine call will be sent 30 minutes prior to your appointment

Connect with a Provider

· Meet our Healthcare Provider on video.

· Our Healthcare Provider will assess your symptoms, explain the diagnosis and treat your condition.

· Prescriptions for medications, lab testing, radiology or a referral to a specialist can be recommended.

· If you require additional care or procedures, we are able to see you in our Urgent Care office
